ROOT CAUSE ANALYSIS – THREE STEPS TO DETERMINE ROOT CAUSES

1. The "5 Whys" Technique:
- A problem-solving method where you repeatedly ask "why?"
- Helps uncover the deeper, root cause of an issue.

2. Fishbone (Ishikawa) Diagram:
- Visual tool shaped like a fish skeleton.
- "Head" represents the main problem.
- "Bones" list potential causes.

3. Pareto Principle (80/20 Rule):
- 80% of effects come from 20% of causes.
- Focus on the most impactful factors for better results.

F.O.C.U.S. Framework for Root Cause Analysis:
- Focus on the actual problem.
- Organize relevant data.
- Create a list of possible causes.
- Understand underlying systems.
- Solve using targeted actions.

6 Steps to Apply Root Cause Analysis Successfully:
1. Clarify the Problem First.
2. Engage Team Members Familiar with the Process.
3. Solve Using Targeted Actions (address root causes).
4. Understand the Underlying Systems (fix processes, not just symptoms).
5. Document and Learn (capture lessons for future reference).
6. Implement One Solution at a Time (monitor results before moving on).

Common Mistakes to Avoid:
- Jumping to conclusions without sufficient data.
- Treating symptoms instead of the real cause.
- Blaming individuals instead of improving system processes.
- Failing to learn from the process (not documenting findings).
